Job Description & Requirements - Specialty: Case Management - Discipline: RN - Start Date: 02/09/2026 - Duration: 26 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Job Details - Position: Registered Nurse (RN) – Case Manager - Specialty: Case Management - Unit: RNCM (1827.735000.12573) - Facility: Providence ElderPlace – MLK - Contract Group: Providence Health and Services – MSP - Start Date: February 9, 2026 - Duration: 26 weeks - Location: 4515 Martin Luther King Jr Way S, Seattle, WA 98108 Schedule - Shift: Day shift – 5x8 - Hours: 8:00 AM – 4:30 PM - Block Scheduling: No - Weekends / Call: None ? Requirements - Active RN license (WA pending accepted) - Minimum 2 years RN experience required - BLS required - Case Management / Care Coordination experience preferred - Experience with geriatric population strongly preferred - Comfortable with community-based visits (mileage reimbursed) - EPIC experience preferred Role Overview - Adult outpatient PACE (Program of All-Inclusive Care for the Elderly) setting - Care coordination, discharge planning, utilization review - Manage participants transitioning from acute care and SNF settings - Interdisciplinary collaboration with physicians and social workers - Primarily onsite with occasional community/home assessments Additional Notes - Locals and true travelers accepted - Candidate must be open to going perm after contract - Laptop and cell phone provided - Mileage reimbursed via Concur - COVID vaccine not required K16 About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
