must be open to permanent employment after the initial contract
period. The Registered Nurse Case Manager establishes a
well-rounded care practice that includes patient assessment, care
planning, implementation, coordination, and monitoring. Ongoing
evaluation ensures medically and financially appropriate care.
Responsibilities include facilitating interdisciplinary team care
coordination and intensive medical management of participants
recently discharged from acute care centers and skilled nursing
facilities. Additionally, this position works closely with the
interdisciplinary team social worker and physician to collect
necessary medical and social information. This ensures cohesive
decisions are made which promote the safety and well-being of
participants. This position is primarily on-site, but there are
occasions when the Registered Nurse Case Manager will need to drive
to a participant's residence for an assessment if needed. Mileage
will be reimbursed for this time. The Registered Nurse Case Manager
will start from the center and leave to the community for visits.
The role focuses on keeping older adults as healthy as possible
while living in the community. All Registered Nurse Case Managers
are assigned a laptop and phone to perform their duties.
Participation in an expense reporting system is required. Candidate
must have experience with care coordination, discharge planning,
and familiarity with CMS criteria. Floating between facilities may
be expected as part of the role. References are required from
previous supervisory roles within the last three years in a
comparable specialty setting. All travelers are expected to float
between facilities. Refusal to float may result in assignment
termination or forfeiture of hours. Requirements Required for
