RESPONSIBILITIES

? Effectively uses readiness assessment, motivational interviewing, goal setting, appreciative inquiry and barrier resolution techniques to assist participants in developing individualized plans for successful behavior change.

? Coaching topics may include: weight management, stress management, nutrition, physical activity, smoking cessation and other health topics identified during the coaching process.

? Conducts health coach sessions in a variety of settings: one-to-one, telephonically, or in group

? Interprets health risk assessment, biometric screening or provider visit results as a basis for health coaching session.

? Successfully recruits participants into individual health coaching or group behavior change programs.

? Understands how to work with individuals on primary, secondary or tertiary prevention education strategies using research proven information.

? Conducts education sessions for client companies on an as-needed basis re: lifestyle education and behavior change.

? Consistently, accurately and correctly documents coaching sessions

? Maintain consistent and accurate coach activity documentation for reporting purposes.

REQUIREMENTS

? Bachelor's Degree in nursing, health promotion, health education, social work or a health-related field. .

? Yoga certification encouraged

? Three to five year's of work experience preferred.

? Competency with basic use of Microsoft Windows and Microsoft Office applications.

? Ability to sit at a computer or interact telephonically for 8 hours at a time.

? Effective time management skills.

? Ability to juggle a variety of tasks.
